Standard. Investigate and report all accidents.  Record accident investigations on appropriate forms. Capture identified findings as hazards in the Base Safety Information System (BaSIS) database.

Description. All classes of Army accidents, as defined herein, are reportable to the safety staff at the unit and/or the Joint Base Safety Office. An Army accident is defined as an unplanned event or series of events which result in one or more of the following:

  1. Occupational illness to Army military personnel or DA Civilian personnel (AF and NAF).
  2. Injury to on-duty DA Civilian personnel.
  3. Injury to Army military  on duty or off duty.
  4. Damage to Army property (including government-furnished materiel, property or equipment provided to a contractor).
  5. Damage to public or private property and/or injury or illness to non-Army personnel caused by Army operations. (the Army had a causal or contributing role in the accident).

Recordable accidents. Certain classes of accidents are recordable and require investigation and formal reporting through the online "Report-It" system and on forms as prescribed by DA Pam 385-40.  All JBLM HQ accidents that result in any of the consequences listed below will be investigated, recorded and forwarded to the JBSO using the criteria indicated below

  1. Disabling injury (including death) to Army military personnel. One or more lost workdays past the date of injury constitute a disabling injury. Included are both on-duty and off-duty operations/activities, regardless of time or place of the accident.


  2. Any occupational illness to Army military personnel, resulting from repeated exposures to chemical, biological, or physical agents in the work environment, will be reported on an AGAR and forwarded to the JBSO regardless of time lost by the soldier.  Examples of these cumulative exposure-type illnesses are:


    1. Noise-induced hearing loss
    2. Asbestosis
    3. Radiation poisoning
    4. Organic solvent exposure
    5. Dermatosis
    6. Silicosis

  3. All injuries requiring medical treatment or occupational illnesses, and fatalities to civilian employees (APF of NAF); compensation claims forms will be completed and processed by either the ICPA (FECA claims) or CPAC (NAF Claims).


  4. Disabling injury (including death) to non-Army personnel or damage to non-Army property as a direct result of Army operations.


  5. All accidents resulting in $5,000 or more damage to Army property.


  6. Any accident involving an Army-operated vehicles or equipment (including General Services Administration (GSA)/TMP fleet vehicles), which results in a disabling injury and/or property damage to the extent of $5,000 or more.


  7. Any on-post contractor accident involving disabling injuries or fatalities, or property damage of $5,000 or more, where no government personnel are injured or government property damaged.

Occupational (on-duty) injuries to military personnel, that do not meet the criteria in for reporting as “Recordable Accidents”, will be reported on DD Form 689. Responsible unit/activity personnel will prepare the DD Form 689, Individual Sick Slip, to accompany the injured soldier to the Emergency Room when reporting for treatment. If injuries are so severe that the time required to prepare the sick slip would jeopardize treatment of the injury, the requirement may be waived.

CLASS D A nonfatal injury or illness results in restricted work, transfer to another job, medical treatment greater than first aid, needle stick injuries, and cuts from sharps that are contaminated from another person’s blood or other potentially infectious material, medical removal under medical surveillance requirements of an OSHA standard, occupational hearing loss; or
A work-related tuberculosis case or
Estimated property damage $20,000 - $50,000
